Job Overview

An exciting opportunity has arisen for a dietitian with a strong interest in nutrition and older adults to work within ABUHB. This post will enable you to develop skills to take a leadership role in the care of older Adults as part of the Community dietetics team. The successful candidate will take the lead of caring for patients within our 6 community hospitals and the care of patients within care home settings across ABUHB. This will include patients requiring a wide range of conditions affecting their nutritional needs with nutrition support and care of patients with diabetes forming the larger part of the case load.

The Community dietetics team are an enthusiastic dietetic team comprising of over 30 staff members working within primary care, community settings, public health & home enteral feeding.

The successful candidate will lead on the development, delivery & evaluation of nutritional care provided to older adults within all care homes and within our 6 community hospitals in ABUHB.

Onboarding via Annex 21 can be considered for applicants who require developmental support, they will be will appointed at 75% of a band 7 for 12 months.

The post holder will work closely with public health and primary care team leads in addition will ensure ongoing cost effective and appropriate prescribing of oral nutritional supplements within community.

The role will support services pan Gwent & will incorporate both face to face and elements of home working.

Main duties of the job

The post holder will be expected to work independently, offering one-to-one dietetic care, group sessions and provide expert support as part of a wide multi-disciplinary team. The successful candidate will work towards leading in the enhancement of dietetic services aimed at older adults with co-production with a wide variety of stake holders.

The successful individual will incorporate an in-depth knowledge of the management of patients in this specialist area and will be instrumental in leading on the development & use of evidence-based principals in future older adult care pathways. The post holder will promote & co-ordinate a specialist & versatile service that can be delivered via both face-to-face support & virtually. A special consideration to Dementia support will play a key part within various aspects of this role.

The post holder will have a keen interest in leadership skills, enhanced communication skills & remarkable organisational & data analysis skills.

The post holders base is Ty Gwent in Cwmbran & travel to different sites to deliver interventions is an essential part of this role.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Qualifications

Essential criteria

  • BSC / MSc / PGDip Dietetics
  • Dietetic Clinical Educators Course
  • Certificate in leadership or Masters module in leadership
  • HCPC registration

Desirable criteria

  • Advanced Educators Course
  • Masters module in specialist area
  • Advanced motivational interviewing/behaviour change training

Skills

  • Excellent written communication skills
