Livewell Southwest is seeking a motivated Community Support Worker to join their Community Learning Disability team in Plymouth, working 37.5 hours per week. This role involves supporting individuals with learning disabilities in accessing essential healthcare services, advocating against health and social care inequalities, and promoting their independence. The successful candidate will be part of a multidisciplinary team that focuses on person-centered care and will engage with individuals in various settings, including their homes and community services.

The ideal candidate should be adaptable, possess strong computer skills, and have a commitment to the values of Livewell Southwest. While previous experience with individuals with learning disabilities is advantageous, it is not required. Responsibilities include assisting with health checks, reporting concerns, implementing treatment packages under supervision, and contributing to service user evaluations. The role may necessitate flexible working hours, including evenings and weekends, as well as travel within Plymouth and surrounding areas.
