The role of the Palliative Specialist Nurse Band 6 is to provide holistic patient centred care for the patients within the hospital who are palliative and/or end of life. It is a pivitol role to ensure palliative patients are getting the right care they need. the service is receiving very high numbers of referrals with last month receiving 174, there is a clear correlation between the SPCT team being involved and the 5 priorities of care document being completed which provides higher quality holistic care when completed, ensuring as a service we can meet the needs/wants and wishes of our patients.
